Nerf Wars Permission Form


    This program is limited to 25 Tweens/Teens (grades 4 - 12).  Permission forms are required and will be updated annually in September.

  3. Parent / Guardian permission*

    I give permission for my child (4 -12) to attend the Nerf Wars at the City of Huron, Huron Public Library, from 5:30 pm to 7:00 pm. I understand that my child will not be allowed to exit and re-enter the program. Library doors will open before the games at 5:25 pm and locked at 5:35 pm. Latecomers will not be admitted. I must pick up my child from the library promptly at 7:00 pm. I understand that the library is not open during Nerf Wars for parents to stay. 

    While noise level rules will be more relaxed, the standard library rules of conduct Rules of Conduct still apply. In addition, I understand that if my child is not behaving in a manner compliant with the "Rules of Nerf Wars", I may be contacted before the conclusion of the program. I agree, if contacted, I will promptly come for my child. 

    I confirm that my child is in 4th-12th grade. I agree not to hold the City of Huron, Huron Public Library, responsible for any accidents or mishaps which may involve my child. If my child should become seriously ill or injured, I authorize the staff of the Huron Public Library to arrange for any emergency medical care required.

    By checking the “I agree” box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.

  4. Rules of Nerf Wars

    • Eye protection (i.e. goggles) is a must. 
    • Stay within the public areas of the library. 
    • NO shooting your opponent within 10 feet of each other; no shots above the shoulders. 
    • NO shooting computer screen monitors or going behind circulation or staff desks. 
    • NO climbing or pushing shelving units. 
    • NO standing on top of or under furniture; no moving furniture. 
    • NO modifications of the Nerf guns. 
    • NO hitting, pinching, biting, kicking, pushing, bullying, striking out, or throwing guns; 
       if you can’t do it in gym class you can’t do it here. 
    • We are not responsible for lost or misplaced Nerf bullets or guns.  
    • Following the games, pickup is REQUIRED
    • Follow instructions of Library staff & volunteers.
